Chairs, Caulk, Form Retarder, Shims, Gaskets: They will send you to a local distributor to order.
Form Oil: Update: After testing we have found that using NO form oil is the best pratice. We use a few coats of wax on our sink mold and they pop out better than they ever did when we oiled them. Same for form sides. We use Rain -X on or form bases.
Fiber Optics: Everything you need to do fiber optic tops. Very informative website.
